Question Banks in Survey Instruments

Question banks are groups of questions within templates that allow for survey instrument modification while facilitating consistency for reporting.  Not all templates have question banks attached.  However, if you think a template should, please contact your State Admin or PFT leader.

Adding a Question Bank Item
While creating or editing a survey instrument,

  1. Under the Questions section, select the question bank group you wish to attach questions to.

  2. Click on the “Available Questions” dropdown.

  3. Select a question you would like to add.

  4. Click Attach Question Button.

  5. Repeat steps 2-4 until all the question bank items you want are added.

  6. Click Create Button or Save Button at the bottom to save changes.


Deleting a Question Bank item
While creating or editing a survey instrument,

  1. Click on the question item with a prefix of “Question Bank Group”.

  2. Click on the Delete Symbol to the right of the question bank item.

  3. Click Create Button or Save Button at the bottom to save changes.
